Ready for Medicare's Ambulatory Specialty Model
Beginning January 1, 2027, spine, pain, PM&R, orthopedic, and neurosurgery specialists in selected markets are financially accountable for the quality and cost of low back pain care under Medicare's mandatory Ambulatory Specialty Model. Payment adjustments of up to ±9% on Medicare Part B claims will depend on what the documentation shows. ReCapture is building ASM documentation-readiness support — mapping each note to the ASM measures (functional status change, depression screening and follow-up, BMI follow-up, high-risk medication review, and primary care coordination) and flagging gaps for physician review — on top of the denial-recovery engine already in production today.
What ReCapture Does
- In development: documentation mapping to ASM low back pain measures (Q220, Q134, Q128, Q238)
- In development: gap flags to support ASM quality reporting
- Physician-board-authored logic, updated as CMS rulemaking evolves
- Reporting for service-line leaders and quality teams, built on ReCapture's production denial-recovery engine
Shared Responsibility
ReCapture is building ASM measure mapping to flag what is missing from the documentation. Whatever the tooling ultimately covers, it does not replace the treating clinician's responsibility for complete, accurate documentation.
ReCapture supports ASM documentation readiness. It does not guarantee compliance or improve your quality score.
Verified Facts About ASM
- ASM is a mandatory CMS Innovation Center model with five performance years, January 1, 2027 through December 31, 2031.
- The low back pain cohort covers anesthesiology, interventional pain management, neurosurgery, orthopedic surgery, pain management, and PM&R.
- Payment adjustments on Medicare Part B claims are two-sided: up to ±9% in the first performance years, rising to ±12% in the final year.
- Quality and cost are each weighted 50%. Improvement activities and interoperability can deduct points.
- Low back pain quality measures: Q238 (high-risk medications in older adults), Q134 (depression screening and follow-up), Q128 (BMI screening and follow-up), and Q220 (functional status change).
- The first reporting deadline is March 31, 2028, for performance year 2027.
Documentation Integrity
ReCapture never adds clinical facts that are not in the record. It organizes what the physician documented into the structure each payer requires, and flags what's missing for the physician to review. The record remains the physician's.
